Aims of the CHCC Pharmacy
Provide Affordable Prescription and Over the Counter Drugs for Patients
The CHCC strives to offer customers the lowest prices possible on prescription drugs and is currently exploring discounted drug arrangements through federal programs such as the Medicaid Drug Rebate Program and the 340B discounted drug program administered by the Health Resources and Services Administration (HRSA). The pharmacy offers a wide range of pharmaceuticals for our customers at the lowest prices possible.

Improve Patient Compliance
Current health literature describes patient non-compliance as an extremely complex web of factors tied up in patient and physician behavior, and the design of the health system itself. The asset of a pharmacy for outpatient services which functions within the CHCC's health system eases communications betweeen outpatient pharmacists and CHCC doctors to assess patient adherence.

Reduce Readmissions
Research shows that beginning long-term medication therapy while the patient is still admitted may improve adherence to those medications after the patient is discharged from the hospital. Creating an effective "meds to beds" program at the hospital and reducing the rate of patient readmissions is one goal of the CHCC outpatient pharmacy.

Improve Medication Reconciliation
Another critically important health system-related factor of patient compliance is ensuring that accurate listings of patient medications is available to medical providers treating a patient. This is important to ensure that the right medications are prescribed to the patient at the right time and aren't duplicated. Sharing data and using the same records systems between the outpatient pharmacy, clinics, and hospital improves the CHCC's ability to conduct medication reconciliation.
