How can I request a copy of my medical records?
To request your medical records, you need to submit a written request to the Medical Records Department. You can download the "Authorization to Release Medical Information" form from the CHCC website, complete it, and ensure it's signed by you or your legal representative. Duplication fees may apply and must be paid in advance. For assistance, contact the Medical Records office at (670) 236-8356 during regular business hours.

The Immunization Program aims to prevent vaccine-preventable diseases by increasing and maintaining high immunization coverage. Services include vaccinations, temporary health certificates, community outreach and education, and enforcement of public health laws.
