Nephrologist
Salary
$280,500 P/A ($135/hr) (the CNMI enjoys one of the lowest tax burdens in the U.S.; Salary is equivalent to that of a $425K salary in San Francisco and New York)
The Practice
- Manage hemodialysis unit with 100+ patients; 3 shifts/day, 27stations (2 iso) with Braun machines
- Manage peritoneal dialysis program
- Coordinate with Internal Medicine Outpatient service for integration of Medical and Dialysis care
- Readmit returning patients (up to 10 patients ‘off island’ at any given time for medical procedures (themselves/family) or travel.)
- Answer inpatient Nephrology consultation requests (1-3/week avg.)
Schedule
- Round on IP, consults. Usually 0-2 new pts/day (ESRD or consult)
- Clinic if scheduled in PM
- Readmit returning HD patients to unit, paperwork
- Resolve HD/PD pt issues as they arise
- See pts with team
- Lab/data/decision making entered into note by team

Qualifications
- Accepting locums, permanent, and locums to permanent

- Board certification in Internal Medicine required.
- Nephrology fellowship or adequate experience to qualify as Medical Director of a dialysis unit preferred.

Pediatrics

Responsibilities
- Care for patients in the general newborn nursery, inpatient Pediatrics ward, and Ambulatory outpatient clinic in rotation with other pediatricians
- Provide consultation to the Emergency Department when on-call for pediatric patients
- Attend to patients in the Neonatal Intensive Care Unit and Pediatric Intensive Care Unit, including intubations, ventilator management, central line placement and monitoring (both arterial and venous), and special procedures required of the patient.
- Provide ICU level care in accordance with NRP and PALS standards. We are happy to support applicants with their first few procedures if they are just out of training or need a refresher!
- Provide consultative service to the nearby islands of Tinian and Rota, including via phone.
- Attend high-risk deliveries at the request of Labor and Delivery (L&D) personnel and provide resuscitation of the newborn in accordance with NRP.
- Provide medical escort to critically ill pediatric patients referred to off-island facilities if interested
- Participate in hospital committees, Quality Assurance activities, and administrative departmental duties.

Job Details
- Inpatient census typically ranges from 5-15 patients with an average of 9 patients (~3 well babies,~3 NICU babies and ~3 general pediatrics inpatients) to be rounded on daily.
- Typically, 1 ICU admission every 1-2 months, most often severe bronchiolitis, non-fatal submersions, DKA, or post-seizure patients.
- An average 2-5 critically ill newborns per year require medical transport, often due to congenital heart and other defects requiring surgical correction, with MD, RN, and RT, to San Diego, for higher level care on commercial United flights (Rady Children’s Hospital in San Diego is our referral hospital. Specialists there allow us to call, email, and refer our patients to them to determine/provide higher level care needs.)…Pediatricians rotate in providing this service according to physician comfort and to minimize schedule disruptions.
Schedule
- One pediatrician is on call at a time covering NICU, General Pediatrics, Nursery, and PICU.
- 8 inpatient shifts and 10 clinic shifts per 4 weeks, though inpatient/ outpatient coverage is negotiable.
- Inpatient is typically a one-week block consisting of three day shifts (8 AM to 4 PM), one 24 hours shift (8 am to 8 am), and three night call shifts (4 PM to 8 AM).
- Night call shift is a home call with providers responsible for cross-cover and admissions.
- Schedule and staff flexibility allows you to stack shifts so you can have 5-7 days off per month in a row without taking leave to travel!

Respiratory Therapist
Duties
- Performs complete arterial blood gas analysis.
- Provides and implements ventilator support for infants, children and adults
- Performs all modes of diagnostic testing as provided by the department such as Stress Test, Pulmonary Function Test and Holter Monitoring
- Follows established Respiratory Care department policy and procedure manuals.
- Performs cardiopulmonary resuscitation according to established policy.
- Maintains respiratory equipment, including cleaning and re-assembly.
- Documents on patient records and department forms according to established policy.
- Performs quality control measures on all diagnostic equipment in department.
- Performs maintenance protocol and quality control on blood gas analyzers according to department policy and Medicare/CLIA ‘88 regulations.
- Provides respiratory care related duties for off-island medical transports of patients as assigned.
- Performs in-service training and patient instruction as required.

Qualifications
- Must be a graduate of an American Medical Association (AMA) approved school of Respiratory Therapy
- Must possess an A.S. or B.S. degree in Respiratory Therapy and have two years related experience, or any combined equivalent of education and experience: (ie, A.A.S. or A.S. in Respiratory Therapy plus 5 years of related experience).
- Must possess at least two years of experience in critical care in a hospital setting.
- Must possess BCLS or ACLS certification for performing cardiopulmonary resuscitation.
- Must be Certified or Registered by the National Board of Respiratory Care.

OBGYN
Job Description
CHCC has 4 labor and delivery rooms, a general OBGYN ward (for postpartum, antepartum, post operations, etc), and an outpatient clinic in the hospital. There are two operating rooms shared by OBGYNs and other surgeons. There is usually midwife coverage on labor and delivery for vaginal deliveries. We have a NICU that is staffed by general pediatricians who do a great job. There is a good amount of high risk obstetrics, often the patients are not stable enough to be transported off island for care so they are managed by us. The department doesn't have a formal relationship with a Maternal Fetal Medicine, so this is a great chance to practice and manage high risk obstetrics.
The outpatient clinic has federal grants to support family planning and breast and cervical cancer screening. When there is adequate staffing, the department will do outreach to the neighboring islands Rota and Tinian (this isn't mandatory, but quite fun and allows you to travel to other islands). There are Radiology services that perform obstetric and gynecologic imaging. The clinic has electronic fetal monitoring capabilities with central monitoring.
Gynecologic surgeries are performed as open abdominal, vaginal or laparoscopic. The equipment for laparoscopy is dated, but available. Often equipment that is common in the US mainland is not available here, like hysteroscopy, but we get creative. There is an above average amount of gynecologic malignancy, so gynecologic care is very much needed.
Duties and Responsibilities
Provides outpatient OB/GYN care (prenatal care including complicated high-risk pregnancies, postpartum care, health care maintenance for women, gynecologic problems, etc.) when not on-call or in OR.
Provides OB/GYN on-call service including consultation (ER and other services) in rotation with other OB/GYN providers. Provides back-up support and general supervision of midwives in L& D.
Cares for hospitalized OB/GYN patients at CHC in rotation with other OB/GYN providers. May be asked also to follow own patients if deemed necessary by the Chairperson.
May provides outer island (Tinian and Rota) OB/GYN service as needed.
Works 40 hours per week on average plus on-call duty and weekend rounds as assigned in rotation with other OB/GYN providers.
Participates in hospital standing committees, CME and Quality Assurance activities.
Performs other related duties as assigned.

Who Are We?
The medical staff of the Commonwealth Health Corporation provide medical care for a diverse multi-cultural community who reside on three islands that make up the U.S. Territory that is called the Commonwealth of the Northern Mariana Islands (CNMI). Our psychiatrists provide inpatient, outpatient, telemedicine and consultation services to the 50,000 people who call CNMI home.
Where Are We?
CNMI is part of an island chain located north of Guam in the western pacific. The three inhabited islands are Saipan, Tinian and Rota. Saipan is home to 85% of the population and the territory’s only hospital. Psychiatric care is provided on Tinian and Rota by telemedicine and scheduled inperson clinic days. The islands are rich in history from the arrival of pacific islanders through World War II battles
Who Works Here?
The medical staff is as diverse as the island’s population. The staff include physicians, nurse practitioners and physician assistants from all ages and backgrounds. Many have just completed training and take advantage of the loan repayment program. Others are at the end of their careers and are looking for adventure. Some came just for a break from practicing on the mainland and decided to stay.
